The P-64 came about after being selected as the winner of a design competition instituted to field a replacement for the TT-30, a Russian pistol designed by Fedor Tokarev. Though the original winner was chambered for the .380 ACP cartridge, the pistol was eventually re-chambered for the 9x18mm Makarov cartridge. Both versions of the pistol supported 6-round magazines.
